What is it?

  • a clinic for diabetic patients in a group setting, with a provider and a trained facilitator (African American health conductor of Spanish speaking promotora)
  • a provider visit with management of diabetic meds., labs, and referrals
  • interactive diabetes curriculum including lifestyle management. Patients learn a lot from each other.
  • a major source of support: breaks isolation and builds community
  • meets every 2 weeks for 6 visits

    • Pittsburg for Bay Point patients (Spanish)
    • West County (English and Spanish)

    • in ccLink ORDERS, type “diabetes group”
    • fill in the Site, Language, and Comments - click "Accept."
    • your referral goes to a Workqueue, and the patient will receive a phone call to schedule the group appointments.
